When scheduling vaccines, all patients must have a consent form filled out when they arrive to be vaccinated.  Click on the blue button below to print a consent form and fill it out before coming to the pharmacy the day of your vaccination.  If you are receiving a Covid vaccine and another vaccine, please fill out both consent forms.   We recommend that all minors have an adult present. ADDITIONAL COVID BOOSTER INFORMATION:

PFIZER and MODERNA
As of 4/18/23, the FDA amended the emergency use authorizations of the Moderna and Pfizer Covid-19 vaccines to simplify the vaccination schedule for most individuals.  This action included authorizing the current bivalent vaccines to be used for all doses administered to all indivudials 6 months of age or older.  The monovalent Moderna and Pfizer Covid-19 vaccines will no longer be given.

If you have any questions about receiving a Covid-19 vaccination, you should talk to your healthcare provider about your medical condition(s) and whether getting an additional dose is appropriate for you.
